Dr. Nase,
I'm thinking of starting Helen Sher's system. I am also considering using a combo of Clonidine/Remeron along with RLT, IPL and Yag. Can you provide me with your input on Helen Sher's system? I know you were involved with her at some point. What is the best course of action for me? I have redness on my nose/cheeks and the flushing/blushing is absolutely brutal. Thanks. |

Hi Del, Clonidine and Remeron are generally very good for rosacea. They act centrally in the brain to reduce flushing responses and peripherally in the facial skin to constrict facial blood vessels (mild constriction). I also think the combination of IPL and YAG is an excellent form of treatment for hyperreaction blood vessels and skin. Yes, I did collaborate with Helen for a while and learned quite a bit. The real secret to her system is focused on being extra gentle to the skin. Over time, this allows the skin to calm down and inflammation to regress. If I had to focus on just a couple treatments you mentioned I would use the oral medications and laser treatments first as they are the best treatments for this condition. _

If you have low blood pressure you need to be monitored because both meds cause low blood pressure. If that is the case, I would take subpressor doses of both medications. Subpressor refers to its ability to work without altering blood pressure and heart rate. (1) Remeron (Mirtazapine) works extremely well at low doses. I would start off with 15 mgs right before bedtime. (2) I would combine Remeron with 1mg Clonidine patches (Catapres) This is a superb treatment combo for rosacea. Evaluate your blood pressure, heart rate and effectiveness for 8 weeks. If everything comes back normal I would work up to 30 mgs of remeron and 2 mgs of the Catapres patch. I hope that helps.
